Frequently Asked Questions

What is the difference between a CGM and a glucometer?
A glucometer gives a precise reading at a moment the patient chooses. A CGM samples interstitial glucose continuously, producing a curve that reveals overnight lows and post-meal spikes a fingerstick regimen would never capture. Many programs use both — CGM for insulin-treated patients, fingerstick meters for the rest.
Which CPT codes apply to CGM?
CGM has its own codes: 95249 for patient-owned sensor startup and training, 95250 for provider-owned CGM with at least 72 hours of recording, and 95251 for the clinician’s analysis and interpretation. Can we bill CGM and RPM in the same month?
Potentially, when the services are distinct and the time is not counted twice. The rules are specific and payer policy varies, so confirm current CMS guidance and your payer’s policy before billing both for the same patient in the same period.
Do we have to change which CGM we prescribe?
No. The intent is to bring the data your team already generates into one clinical workflow, not to make you standardize on different hardware.
What is time in range and why does it matter?
Time in range is the percentage of the day a patient’s glucose stays within their target band. It has become a central diabetes metric because it captures variability and hypoglycemia risk that an A1c average conceals.
