Elation Health makes a primary care EHR used by independent and value-based practices.
HealthArc connects through Elation’s FHIR R4 APIs with SMART on FHIR (OAuth 2.0). Elation reviews and registers each application, starting in a sandbox. Production access follows after testing and needs written data-sharing authorization from your practice.
Diagnoses, insurance and visit history from Elation feed HealthArc’s eligibility screening, so your team starts with a list of patients who qualify for CCM, RPM or APCM instead of a manual chart review.
Monthly care-management summaries and time logs are produced for every enrolled patient. Write-back to Elation is confirmed during scoping and depends on the access Elation grants.
Readings from 40+ connected devices sit next to the patient’s problem list and medications, so a high blood pressure alert is read against the full chart.
HealthArc tracks time and device days against each CPT and HCPCS code and produces a monthly billing record for every patient and program.
The same connection serves RPM, CCM, APCM, PCM, RTM, BHI, TCM, MTM and PIN. Adding a program later doesn’t mean a new integration project.
Data moves between Elation and HealthArc under a business associate agreement, encrypted in transit and at rest.
HealthArc programs by specialty: Primary care, Cardiology, Nephrology, Endocrinology, Pulmonology.
We confirm the programs you want to run, the providers and locations involved, and who owns clinical work and billing on your side. The HealthArc agreement and business associate agreement are signed before any data moves.
Complete Elation’s Developer Sandbox / Partnership form. In the description, say you’re requesting FHIR API access for integration with the HealthArc platform.
An Elation representative contacts you and reviews the application before registering it. You may be asked for:
The Elation Health setup guide covers prerequisites, every field that moves each way, and who does what.
Yes. HealthArc connects to Elation through its FHIR R4 APIs with SMART on FHIR. Elation registers the application in a sandbox first, then in production once your practice authorizes data sharing.
All nine programs HealthArc supports: Remote Patient Monitoring, Chronic Care Management, Advanced Primary Care Management, Principal Care Management, Remote Therapeutic Monitoring, Behavioral Health Integration, Transitional Care Management, Medication Therapy Management and Principal Illness Navigation.
It can. Elation says additional API fees may apply depending on your practice’s agreement with Elation. Ask your Elation representative during onboarding.
Elation registers new applications in its sandbox so integration testing happens without production patient data. Production credentials are issued separately after testing.
Elation reviews each request before issuing credentials, so the timeline depends on that review and on your practice’s production authorization. HealthArc’s own configuration and testing take days once credentials arrive.
Cloud EHR and practice management for specialty and multi-specialty practices.
EHR for skilled nursing, senior living and other post-acute care settings.
